Important Details:

  • All currently enrolled CCM patients in your current CCM program must transition to ChartSpan.
  • Any patients that have not had a CCM claim submitted by you in the past 60 days are not considered “enrolled” for the purpose of this initiative. You must supply an accurate list of all enrolled CCM patients billed in the past 60 days.
  • The six-month window begins on the first day of the first full month in which enrollment has begun and ends on the last day of the last full month in which enrollment occurred.
  • ChartSpan must have full access to your EHR to conduct an accurate and complete Technical Assessment and Client Readiness Assessment, prior to contracting.
  • If ChartSpan fails to increase the size of your CCM program by 3x, under the terms of the agreement, ChartSpan will provide you a pro-rata credit of those fees over the remaining 30 months of the agreement.
  • You must follow all ChartSpan enrollment and clinical best practices.
  • All participating providers must participate through the initial term of the agreement.
  • Enrollment solicitation for non-enrolled patients must not have occurred in the past 12 months. You must supply an accurate list of all patients you attempted to enroll in the past 12 months.
  • The 3x-enrolled patient benchmark number agreed upon will be put into the agreement.
  • Unenrollments will not factor into the 3x enrollment contractual benchmark.
  • This offer is only applied to standard 36-month agreements.
  • If you wish to transfer to ChartSpan Advanced Primary Care Management during the term of the Chronic Care Management agreement, you may do so. However, this transition negates the 3x guarantee window.
  • ChartSpan reserves the right not to finalize an agreement with any client.
  • The 3x guarantee is only applicable for agreements signed between March 4, 2025 and June 30, 2025.
